If a Middle School student plans on participating in a High School sport while in Middle School, they will need to complete BOTH forms.
KHSAA has revised the Sports Physical form.
- Pages 1-3 are to be completed and returned to the school (Healthcare provider signature required on pg. 3).
- Pages 4-6 are kept on file at the Healthcare Provider’s office and not returned to the school.
