If you are sending medication to school with your child, the Medication Consent & Release (2 pages) must be filled out for each medication being sent.
You can utilize a feature using Adobe to type the information on the forms prior to printing them.

Please bring the following items with you for registration:
- All Parish Sponsored Families
- Please bring your Parish Voucher. This is very important. If you do not have this, you will be charged the full Non Catholic tuition rate
- New Families (Grade 1-12)
- Baptismal certificate (if Catholic)
- OCS will request student records from previous school
- New Familieis (Kindergarten)
- Baptismal Certificate (if Catholic)
- Birth Certificate - original (we will make a copy for school file)
- Social Security Card - original (we will make a copy for school file)
- Kentucky Medical Examination Form - dated within 12 molnths prior to admission to school
- Kentucky Immunization Form
- Kentucky Eye Examination Form
- Families who have not Pre-Registered
- Completed Pre-Registration Form with $50 pre-registration fee
